補(bu)腎水以(yi)治命門火(huo)衰

命(ming)門(men)(men)火衰(shuai)是較(jiao)為(wei)嚴重的腎陽(yang)(yang)(yang)(yang)(yang)虛衰(shuai)證。臨(lin)床可見身寒、怕(pa)冷、腰酸、眩暈(yun)、夜(ye)尿頻多,或有嘔惡腹脹、反胃噎膈、五更泄瀉、浮腫陽(yang)(yang)(yang)(yang)(yang)痿、滑精等。張氏認為(wei)這(zhe)多因元(yuan)陽(yang)(yang)(yang)(yang)(yang)不足,先天(tian)稟賦陽(yang)(yang)(yang)(yang)(yang)衰(shuai),或勞傷陽(yang)(yang)(yang)(yang)(yang)氣,或久病傷陽(yang)(yang)(yang)(yang)(yang)所致(zhi)。一般以(yi)溫(wen)補腎陽(yang)(yang)(yang)(yang)(yang)立方(fang)施治(zhi),而張氏則以(yi)“益火之(zhi)源”,大培(pei)腎中之(zhi)元(yuan)陽(yang)(yang)(yang)(yang)(yang),用右歸(gui)丸(wan)治(zhi)之(zhi)。方(fang)中雖用附子、肉桂,收溫(wen)補命(ming)門(men)(men)、扶(fu)植(zhi)陽(yang)(yang)(yang)(yang)(yang)氣之(zhi)功,而方(fang)中以(yi)熟地(di)黃為(wei)君(jun),熟地(di)黃能“大補血衰(shuai)”,“滋(zi)培(pei)腎水”,其在(zai)“陽(yang)(yang)(yang)(yang)(yang)以(yi)陰(yin)為(wei)基”,“陰(yin)中求陽(yang)(yang)(yang)(yang)(yang)”。雖知陽(yang)(yang)(yang)(yang)(yang)能生(sheng)陰(yin),但其更注重“陰(yin)亦能生(sheng)陽(yang)(yang)(yang)(yang)(yang)”。補益命(ming)門(men)(men)火重在(zai)填補真(zhen)陰(yin)之(zhi)腎水。

培元(yuan)氣(qi)養精血(xue)以治大虛(xu)

元(yuan)(yuan)氣充(chong)足,生命旺(wang)盛;元(yuan)(yuan)氣虧虛(xu),百病(bing)皆生,如虛(xu)損、勞倦內傷及其他雜病(bing)。癥(zheng)見困倦、怠(dai)惰(duo)嗜臥、懶于言語、脈(mo)緩而大。張氏以培(pei)補元(yuan)(yuan)氣、扶本為(wei)主(zhu)。而對于“元(yuan)(yuan)氣不(bu)足而虛(xu)熱不(bu)已者”,強調“必(bi)有大補元(yuan)(yuan)煎,庶乎從之自愈(yu)”。對于元(yuan)(yuan)氣大虛(xu)者,雖有寒邪亦(yi)(yi)不(bu)可攻,單(dan)培(pei)根本,正復邪將自散;或真(zhen)寒假熱等證,“氣血(xue)(xue)大壞,精神失守危急等證亦(yi)(yi)用此(ci)法”。方(fang)以人參為(wei)首(shou),加山藥(yao)、熟地黃、杜仲、當歸(gui)、山茱萸、枸(gou)杞子均以填精養(yang)血(xue)(xue)之要藥(yao),炙甘(gan)草“隨血(xue)(xue)藥(yao)入血(xue)(xue)”。

引火歸原以治格陽證

格(ge)陽(yang)之(zhi)(zhi)證是(shi)體(ti)內陰(yin)寒之(zhi)(zhi)邪過盛,陽(yang)氣格(ge)于上(shang)而(er)出(chu)現下(xia)真寒、上(shang)假熱(re)的證候(hou),出(chu)現煩熱(re)躁動、大吐大衄、六脈細脫、手足厥(jue)冷等危象。其多(duo)由傷陰(yin)引起,以及(ji)寒涼所(suo)逼(bi),以致寒盛于下(xia)而(er)格(ge)陽(yang)于上(shang),出(chu)現下(xia)寒上(shang)熱(re)之(zhi)(zhi)證。張氏用鎮飲煎治之(zhi)(zhi),方中用附、桂溫補命(ming)門之(zhi)(zhi)陽(yang),并以熟地黃大補腎水,牛膝補髓(sui)填精益陰(yin),澤(ze)瀉行(xing)其降下(xia)之(zhi)(zhi)功,從而(er)能(neng)使腎水精血(xue)有所(suo)復,格(ge)拒之(zhi)(zhi)陽(yang)能(neng)歸(gui)原。這(zhe)與通常用熱(re)藥(yao)而(er)反佐法頗(po)不相同(tong),亦是(shi)張氏之(zhi)(zhi)新(xin)意。

益腎精治(zhi)腎虛喘(chuan)逆(ni)

張(zhang)氏(shi)治(zhi)氣(qi)(qi)(qi)(qi)(qi)喘(chuan)(chuan)(chuan)重虛(xu)(xu)實(shi)之(zhi)辨,“實(shi)喘(chuan)(chuan)(chuan)者(zhe)有(you)邪(xie),邪(xie)氣(qi)(qi)(qi)(qi)(qi)實(shi)也(ye),虛(xu)(xu)喘(chuan)(chuan)(chuan)者(zhe)無邪(xie),元(yuan)氣(qi)(qi)(qi)(qi)(qi)虛(xu)(xu)也(ye)”。而(er)元(yuan)氣(qi)(qi)(qi)(qi)(qi)虛(xu)(xu)之(zhi)喘(chuan)(chuan)(chuan)也(ye)有(you)不(bu)同之(zhi)分,脾肺氣(qi)(qi)(qi)(qi)(qi)虛(xu)(xu)之(zhi)喘(chuan)(chuan)(chuan),則中、上二焦,化源(yuan)未虧(kui),證見(jian)“慌張(zhang)氣(qi)(qi)(qi)(qi)(qi)怯,聲低息短(duan)”;肝腎(shen)(shen)氣(qi)(qi)(qi)(qi)(qi)虛(xu)(xu)則病(bing)出下焦,可見(jian)“氣(qi)(qi)(qi)(qi)(qi)短(duan)似(si)喘(chuan)(chuan)(chuan),呼吸(xi)促急”;或出現“若氣(qi)(qi)(qi)(qi)(qi)欲斷(duan),提之(zhi)若不(bu)能(neng)升,吞之(zhi)若不(bu)相(xiang)(xiang)及(ji),勞(lao)動則甚”,“脈息微(wei)弱無力”,此為(wei)“本末俱病(bing),肺腎(shen)(shen)之(zhi)氣(qi)(qi)(qi)(qi)(qi)不(bu)能(neng)相(xiang)(xiang)接續”。張(zhang)氏(shi)認為(wei)氣(qi)(qi)(qi)(qi)(qi)虛(xu)(xu)之(zhi)喘(chuan)(chuan)(chuan)為(wei)氣(qi)(qi)(qi)(qi)(qi)道(dao)噎塞(sai),勢劇(ju)垂危之(zhi)證,決不(bu)可誤作痰逆氣(qi)(qi)(qi)(qi)(qi)滯(zhi)而(er)妄用(yong)牛黃(huang),“蘇合及(ji)青陳枳殼(ke)等,若用(yong)之(zhi),必(bi)速其危矣(yi)。治(zhi)法當以速效其根接助真(zhen)(zhen)氣(qi)(qi)(qi)(qi)(qi)”。對于(yu)肺腎(shen)(shen)兩虧(kui),腎(shen)(shen)不(bu)納(na)氣(qi)(qi)(qi)(qi)(qi)之(zhi)喘(chuan)(chuan)(chuan),治(zhi)以接納(na)真(zhen)(zhen)氣(qi)(qi)(qi)(qi)(qi),方用(yong)黑(hei)錫丹(dan)以鎮攝下元(yuan)或人參(can)蛤蚧散補腎(shen)(shen)氣(qi)(qi)(qi)(qi)(qi),亦可參(can)附湯益氣(qi)(qi)(qi)(qi)(qi)溫陽。張(zhang)氏(shi)重在維護真(zhen)(zhen)氣(qi)(qi)(qi)(qi)(qi)之(zhi)根,益精(jing)化氣(qi)(qi)(qi)(qi)(qi),精(jing)足(zu)而(er)氣(qi)(qi)(qi)(qi)(qi)自充,使真(zhen)(zhen)陰不(bu)匱而(er)陽氣(qi)(qi)(qi)(qi)(qi)有(you)根。這與一般(ban)的(de)納(na)氣(qi)(qi)(qi)(qi)(qi)歸腎(shen)(shen)治(zhi)法有(you)所不(bu)同。

救(jiu)陽補陰治(zhi)非風脫(tuo)證

張氏認(ren)為時(shi)人所謂中(zhong)風(feng),本由(you)“內傷積損頹敗而(er)然,原非(fei)外感(gan)風(feng)寒所致(zhi),多(duo)由(you)于(yu)氣(qi)(qi)脫(tuo)造成非(fei)風(feng)卒(zu)倒等(deng)癥(zheng)。證(zheng)見“忽爾昏憒(kui),卒(zu)然仆(pu)倒”等(deng),則屬“陽(yang)(yang)氣(qi)(qi)暴脫(tuo)”之(zhi)候(hou),認(ren)為只要(yao)察其無痰氣(qi)(qi)阻塞,便當急用大(da)劑參(can)附峻補元(yuan)氣(qi)(qi)。由(you)于(yu)非(fei)風(feng)一證(zheng)“先傷五臟(zang)之(zhi)真(zhen)陰”,而(er)后才損傷元(yuan)氣(qi)(qi)。故有“陰虧于(yu)前而(er)陽(yang)(yang)損于(yu)后,陰陷于(yu)下而(er)陽(yang)(yang)乏(fa)于(yu)上,以致(zhi)陰陽(yang)(yang)相失(shi),精氣(qi)(qi)不交”,而(er)卒(zu)中(zhong)病(bing)發(fa)。因此在(zai)急用參(can)附扶陽(yang)(yang)氣(qi)(qi)之(zhi)時(shi),又隨用地黃、當歸、枸杞子等(deng)填(tian)補真(zhen)陰,以培(pei)本。從(cong)而(er)使精血充(chong)足,化為陽(yang)(yang),充(chong)養人體病(bing)自(zi)復。張景岳(yue)此法之(zhi)意(yi),同時(shi)也(ye)充(chong)實(shi)了河間(jian)中(zhong)風(feng)證(zheng)治內容。

總之(zhi),任何疾(ji)病(bing),證(zheng)(zheng)勢發展(zhan)到(dao)危(wei)急(ji)關頭(tou),應先(xian)留人(ren)后治病(bing),這是中醫處(chu)理急(ji)證(zheng)(zheng)的一個基(ji)本(ben)原則。決斷急(ji)證(zheng)(zheng)之(zhi)兇吉,不在病(bing)邪的輕重,而在元(yuan)(yuan)氣(qi)之(zhi)存亡,元(yuan)(yuan)氣(qi)不傷。元(yuan)(yuan)氣(qi)傷敗,邪雖微亦多亡。病(bing)證(zheng)(zheng)之(zhi)危(wei)急(ji),往往是陽(yang)氣(qi)將(jiang)脫,陰(yin)(yin)血(xue)將(jiang)竭;或陰(yin)(yin)陽(yang)氣(qi)血(xue)俱傷,元(yuan)(yuan)氣(qi)即離(li)。凡搶救急(ji)危(wei)重證(zheng)(zheng),法宗景岳,調(diao)補腎門陰(yin)(yin)陽(yang),大有裨益。
