人參出自《神農(nong)本草經》,其性(xing)微溫,味甘(gan)微苦,性(xing)稟中和。入脾(pi)、肺二經。善補脾(pi)肺之氣。

一、臨證:

1.1益氣生津復脈:本(ben)品味甘入心(xin)(xin),補(bu)(bu)益(yi)心(xin)(xin)氣(qi)(qi)(qi),氣(qi)(qi)(qi)能生(sheng)津(jin)(jin)化血(xue)(xue)(xue),氣(qi)(qi)(qi)旺而生(sheng)陽,血(xue)(xue)(xue)生(sheng)而和(he)陰,功主益(yi)心(xin)(xin)氣(qi)(qi)(qi)生(sheng)津(jin)(jin)液,為氣(qi)(qi)(qi)中之(zhi)血(xue)(xue)(xue)藥,心(xin)(xin)氣(qi)(qi)(qi)得補(bu)(bu),心(xin)(xin)血(xue)(xue)(xue)得行,脈(mo)亦(yi)自復。《神農本(ben)草經》曰:“補(bu)(bu)五臟,……止驚(jing)悸(ji)(ji),除(chu)邪氣(qi)(qi)(qi),明目,開心(xin)(xin),益(yi)智。”《本(ben)草匯言》曰:“人參(can),補(bu)(bu)氣(qi)(qi)(qi)生(sheng)血(xue)(xue)(xue),助(zhu)精養神之(zhi)藥也。”“驚(jing)悸(ji)(ji)、怔忡、健忘、恍惚,以此寧之(zhi)。”方見炙甘草湯,主治心(xin)(xin)氣(qi)(qi)(qi)血(xue)(xue)(xue)陰陽俱虛(xu)證。

1.2補氣驅寒治胸痹:本品(pin)味(wei)甘而(er)補(bu)心氣,味(wei)苦(ku)主泄而(er)暢達心氣,性(xing)溫而(er)溫煦心氣以驅(qu)陰(yin)(yin)(yin)寒(han),甘苦(ku)溫相合,補(bu)益心氣以益陽(yang)(yang),暢達心氣而(er)通陽(yang)(yang),驅(qu)除陰(yin)(yin)(yin)寒(han)而(er)開閉,主心陽(yang)(yang)虛陰(yin)(yin)(yin)寒(han)相乘之(zhi)(zhi)痹(bi)(bi)證(zheng)。《本草(cao)思(si)辨錄》曰:“蓋(gai)脈生于營(ying),營(ying)屬心,心體陰(yin)(yin)(yin)而(er)用陽(yang)(yang),惟沖和(he)煦育之(zhi)(zhi)人參能(neng)補(bu)之(zhi)(zhi)。”《本草(cao)經疏》:“蓋(gai)臟雖有五,以言乎生氣之(zhi)(zhi)流通則(ze)(ze)一(yi)也,益真(zhen)氣,則(ze)(ze)五臟皆(jie)補(bu)矣。邪氣之(zhi)(zhi)所(suo)以久(jiu)留而(er)不(bu)(bu)去者(zhe),無他,真(zhen)氣虛則(ze)(ze)不(bu)(bu)能(neng)敵,故留連而(er)不(bu)(bu)解(jie),茲(zi)得(de)補(bu)而(er)真(zhen)氣充實(shi),則(ze)(ze)邪不(bu)(bu)能(neng)容。”方(fang)見人參湯,主治虛寒(han)胸(xiong)痹(bi)(bi)證(zheng)。

1.3益肺生陰津:本品益肺(fei)(fei)氣而生(sheng)津,調營衛而和(he)肺(fei)(fei)陰;與麥門冬相用,使(shi)津從氣化而生(sheng),陰從氣化而成,即益肺(fei)(fei)氣而生(sheng)陰津。《用藥(yao)法(fa)象(xiang)》曰:“人參(can)甘溫,能補(bu)肺(fei)(fei)中元氣;肺(fei)(fei)氣旺(wang)則四臟(zang)之氣皆(jie)旺(wang),精(jing)自生(sheng)而形(xing)自盛,肺(fei)(fei)主諸(zhu)氣故也(ye)。仲景(jing)以人參(can)為補(bu)血者(zhe),蓋血不(bu)自生(sheng)、須得(de)生(sheng)陽(yang)氣之藥(yao)乃(nai)生(sheng),陽(yang)生(sheng)則陰長,血乃(nai)旺(wang)矣(yi)。”方(fang)見麥門冬湯,主治虛熱肺(fei)(fei)痿證(zheng)或胃(wei)陰虛證(zheng)。

1.7補益中氣祛濕熱:本品補中(zhong)(zhong)益(yi)氣(qi),使脾胃(wei)之(zhi)氣(qi)各(ge)司其(qi)職而(er)(er)又相(xiang)互為(wei)用,脾不(bu)得(de)生(sheng)濕(shi)(shi)(shi),胃(wei)不(bu)得(de)生(sheng)熱(re)(re),脾陰(yin)和于胃(wei)陽(yang),胃(wei)陽(yang)調于脾陰(yin),陰(yin)陽(yang)平秘,既可(ke)驅(qu)除已成之(zhi)濕(shi)(shi)(shi)熱(re)(re),又可(ke)斷絕(jue)濕(shi)(shi)(shi)熱(re)(re)變生(sheng);與黃(huang)連相(xiang)用,補益(yi)之(zhi)中(zhong)(zhong)以清熱(re)(re)燥(zao)濕(shi)(shi)(shi),清熱(re)(re)燥(zao)濕(shi)(shi)(shi)之(zhi)中(zhong)(zhong)借(jie)正以驅(qu)邪(xie)(xie),補而(er)(er)不(bu)戀邪(xie)(xie),潰(kui)邪(xie)(xie)不(bu)傷正,相(xiang)互為(wei)用,以達邪(xie)(xie)去正復(fu)。方見半夏瀉(xie)心(xin)湯(tang)(tang)中(zhong)(zhong),主(zhu)治(zhi)中(zhong)(zhong)虛(xu)濕(shi)(shi)(shi)熱(re)(re)痞利(li)證;方見生(sheng)姜瀉(xie)心(xin)湯(tang)(tang)中(zhong)(zhong),主(zhu)治(zhi)中(zhong)(zhong)虛(xu)濕(shi)(shi)(shi)熱(re)(re)、痞挾食(shi)滯水(shui)氣(qi)證;方見甘(gan)草瀉(xie)心(xin)湯(tang)(tang)中(zhong)(zhong),主(zhu)治(zhi)狐(hu)盛(sheng)病屬(shu)濕(shi)(shi)(shi)熱(re)(re)型或(huo)中(zhong)(zhong)虛(xu)濕(shi)(shi)(shi)熱(re)(re)痞利(li)重(zhong)證;

1.8補中益氣調陰陽:本品補中(zhong)益氣,既(ji)能(neng)使(shi)脾(pi)(pi)(pi)陰(yin)(yin)以(yi)(yi)濟(ji)胃(wei)陽(yang),又(you)能(neng)使(shi)胃(wei)陽(yang)以(yi)(yi)煦脾(pi)(pi)(pi)陰(yin)(yin),胃(wei)陽(yang)得脾(pi)(pi)(pi)陰(yin)(yin)而不生熱(re),脾(pi)(pi)(pi)陰(yin)(yin)得胃(wei)陽(yang)而不生寒;與(yu)干姜相(xiang)用,使(shi)胃(wei)陽(yang)和協于脾(pi)(pi)(pi)陰(yin)(yin);與(yu)黃(huang)連(lian)相(xiang)用,使(shi)脾(pi)(pi)(pi)陰(yin)(yin)燮理(li)于胃(wei)陽(yang),脾(pi)(pi)(pi)胃(wei)陰(yin)(yin)陽(yang)和調,則寒熱(re)證除。《醫方集解曰:“人參(can)(can)助(zhu)正祛邪。方見黃(huang)連(lian)湯中(zhong),主治(zhi)胃(wei)熱(re)脾(pi)(pi)(pi)寒證以(yi)(yi)脾(pi)(pi)(pi)寒為主;方見干姜黃(huang)連(lian)黃(huang)芩人參(can)(can)湯中(zhong),主治(zhi)胃(wei)熱(re)脾(pi)(pi)(pi)寒證以(yi)(yi)胃(wei)熱(re)為主。

1.9補中益氣卻痰飲:本(ben)品甘(gan)溫(wen),甘(gan)能補(bu)脾益胃(wei),溫(wen)能氣化(hua)水(shui)(shui)液,甘(gan)溫(wen)相用,健(jian)運(yun)(yun)中氣,氣化(hua)水(shui)(shui)液,輸布(bu)津液,運(yun)(yun)化(hua)水(shui)(shui)濕(shi)(shi),水(shui)(shui)濕(shi)(shi)不得(de)留居,不得(de)變生(sheng)痰飲;與(yu)半夏相用,補(bu)益中氣以溫(wen)化(hua)痰濕(shi)(shi),溫(wen)化(hua)痰濕(shi)(shi)以健(jian)運(yun)(yun)脾胃(wei),脾胃(wei)得(de)補(bu),痰飲得(de)平。《傷(shang)寒來蘇集》曰:“人參、大棗補(bu)虛(xu)于余邪(xie)未(wei)平之(zhi)時,預(yu)治(zhi)其本(ben)也(ye),扶正(zheng)驅邪(xie),神(shen)自(zi)安。”方見旋(xuan)覆代(dai)赭湯(tang)中,主治(zhi)中虛(xu)痰飲痞(pi)證。

1.10補中益氣愈虛熱:本品補(bu)中益氣(qi)(qi),使脾(pi)胃(wei)(wei)陰陽(yang)之氣(qi)(qi)調和;與竹茹相用(yong),補(bu)益脾(pi)胃(wei)(wei)以(yi)平虛熱,并使胃(wei)(wei)中邪熱得(de)去,胃(wei)(wei)氣(qi)(qi)得(de)降,職司納降功能。《月(yue)池(chi)人參傳》曰:“人參,生用(yong)氣(qi)(qi)涼,,熟(shu)用(yong)氣(qi)(qi)溫,味甘補(bu)陽(yang),微苦補(bu)陰,如土(tu)虛火旺之病,則宜生參涼薄之氣(qi)(qi),以(yi)瀉(xie)火而(er)補(bu)土(tu),是(shi)純用(yong)其氣(qi)(qi)也(ye)。方見橘皮竹茹湯中,脾(pi)胃(wei)(wei)氣(qi)(qi)虛挾(xie)熱噦(hui)逆證。

1.11補益中氣行脹滿:本(ben)品甘溫補(bu)益(yi)脾(pi)胃(wei)(wei),使脾(pi)能升清(qing),清(qing)氣不壅阻(zu)于中,使胃(wei)(wei)氣能降(jiang)濁,濁氣不填塞于心(xin)下(xia)(xia),清(qing)濁不能搏結心(xin)下(xia)(xia)則(ze)脹滿(man)行(xing)散(san);與厚(hou)樸相用,補(bu)益(yi)之中以行(xing)散(san),行(xing)散(san)不傷正(zheng)(zheng),扶正(zheng)(zheng)不留邪(xie)。《傷寒六(liu)經辨證(zheng)治法》曰:“人參甘草(cao)補(bu)正(zheng)(zheng)和(he)中,俾正(zheng)(zheng)氣實(shi)而邪(xie)自退。”

方見厚樸(pu)生姜半(ban)夏甘草人參湯中,主治脾氣虛(xu)氣滯證。

1.12補肝氣止逆氣:本品大(da)補中氣(qi)(qi)(qi)(qi),健脾益胃,升清以(yi)生化氣(qi)(qi)(qi)(qi)血(xue),滋榮肝(gan)(gan)(gan)氣(qi)(qi)(qi)(qi)肝(gan)(gan)(gan)血(xue)。又(you)能(neng)直補厥陰(yin)肝(gan)(gan)(gan),使肝(gan)(gan)(gan)氣(qi)(qi)(qi)(qi)職司(si)藏血(xue)、疏泄,用陽之能(neng),使肝(gan)(gan)(gan)氣(qi)(qi)(qi)(qi)既能(neng)和(he)調于(yu)內,又(you)能(neng)御邪于(yu)外;與(yu)吳(wu)茱(zhu)萸相用,溫(wen)暖肝(gan)(gan)(gan)之氣(qi)(qi)(qi)(qi),散除陰(yin)寒之氣(qi)(qi)(qi)(qi),肝(gan)(gan)(gan)之氣(qi)(qi)(qi)(qi)得平而(er)不(bu)上(shang)逆。《絳(jiang)雪園(yuan)古方選注》曰(yue):“吳(wu)茱(zhu)萸直入厥陰(yin),招其(qi)垂絕之陽,與(yu)人參(can)震坤合(he)德,以(yi)保生氣(qi)(qi)(qi)(qi)。”

方見吳茱萸湯中,主治厥陰肝寒(han)氣(qi)逆證或陽明胃虛寒(han)證;

1.13益中生津退熱:本品益氣以(yi)生(sheng)津(jin)(jin)(jin),生(sheng)津(jin)(jin)(jin)以(yi)平熱;與石(shi)膏相用,益氣清熱并舉(ju),養(yang)陰(yin)生(sheng)津(jin)(jin)(jin)同行,共(gong)奏益氣生(sheng)津(jin)(jin)(jin),清瀉邪熱。《傷寒來蘇集》曰(yue)(yue):“惟加(jia)人參(can)則(ze)火瀉而土(tu)不傷。”《絳雪園古(gu)方(fang)選注》曰(yue)(yue):“若胃熱經久傷氣,氣虛(xu)不能生(sheng)津(jin)(jin)(jin)者,必(bi)須人參(can)養(yang)正回津(jin)(jin)(jin)。”方(fang)見(jian)白(bai)虎加(jia)人參(can)湯中,主治陽(yang)明熱盛津(jin)(jin)(jin)氣兩傷證;方(fang)見(jian)竹葉(xie)石(shi)膏湯中,主治胃熱津(jin)(jin)(jin)傷氣逆(ni)證。

1.14益膽調氣益氣:本(ben)品補(bu)益膽(dan)氣(qi),使膽(dan)虛得補(bu),膽(dan)氣(qi)復常(chang),并使膽(dan)氣(qi)與(yu)(yu)藥力(li)相合以(yi)驅邪(xie)外出;與(yu)(yu)柴胡(hu)(hu)、黃(huang)芩相用(yong),使膽(dan)氣(qi)有(you)足夠力(li)量驅除邪(xie)熱。《神農百草經百種錄》曰:“人參(can)得天地精英(ying)純粹(cui)之(zhi)氣(qi)以(yi)生,與(yu)(yu)人之(zhi)氣(qi)體(ti)相似,故于人身(shen)無所不(bu)補(bu),非若他藥有(you)偏長而(er)治病(bing)各有(you)其能(neng)也。或仲景傷(shang)寒(han)方中(zhong),病(bing)未(wei)去而(er)用(yong)參(can)者不(bu)少,如小(xiao)柴胡(hu)(hu)、新加湯之(zhi)類,何也?曰:此(ci)則以(yi)補(bu)為瀉之(zhi)法也。”見小(xiao)柴胡(hu)(hu)湯中(zhong),主治少陽(yang)膽(dan)熱氣(qi)郁少氣(qi)證(zheng)。方見柴胡(hu)(hu)加芒硝湯,主治膽(dan)、大(da)腸熱證(zheng)者;方見柴胡(hu)(hu)桂枝湯中(zhong),主治少陽(yang)膽(dan)熱證(zheng)與(yu)(yu)太陽(yang)中(zhong)風(feng)證(zheng)相兼。

1.16益腎和陰陽治煩躁:本品益氣以(yi)(yi)(yi)生(sheng)津,榮腎(shen)以(yi)(yi)(yi)補陰(yin),并(bing)安(an)精神;與茯苓相(xiang)用,安(an)神定志,使陰(yin)以(yi)(yi)(yi)守(shou)陽(yang)(yang),陽(yang)(yang)以(yi)(yi)(yi)護陰(yin),陰(yin)陽(yang)(yang)相(xiang)依,則煩躁(zao)不生(sheng)。《絳雪園古方(fang)選注(zhu)》曰:“人參、甘草下行(xing)以(yi)(yi)(yi)安(an)欲失之真陽(yang)(yang)。”入方(fang)見茯苓四逆湯中,主治(zhi)腎(shen)陰(yin)陽(yang)(yang)俱虛煩躁(zao)證。

1.17益腎陽祛寒濕:本品味甘大補(bu)元(yuan)氣(qi);與附(fu)子相(xiang)用,益氣(qi)溫(wen)陽(yang)(yang)(yang)之中(zhong)以(yi)補(bu)腎陽(yang)(yang)(yang),振奮腎中(zhong)陽(yang)(yang)(yang)氣(qi),祛除內外(wai)寒(han)濕(shi),調榮養衛(wei)(wei),以(yi)舒和(he)肌肉營衛(wei)(wei)之氣(qi)。《傷寒(han)論辨證廣注》曰:“以(yi)人參(can)為(wei)使(shi),而(er)補(bu)其(qi)陽(yang)(yang)(yang),以(yi)益其(qi)元(yuan)氣(qi),散(san)其(qi)陰邪也”《傷寒(han)纘論》日“附(fu)子湯中(zhong)人參(can)助生附(fu),純(chun)用其(qi)溫(wen)補(bu)之力,以(yi)快復渙散(san)真(zhen)陽(yang)(yang)(yang)。”方見(jian)附(fu)子湯中(zhong),主治腎陽(yang)(yang)(yang)虛寒(han)濕(shi)體(ti)痛(tong)證。

1.18益氣回陽生津:本品益(yi)氣(qi)而生(sheng)(sheng)陽(yang)(yang),生(sheng)(sheng)陽(yang)(yang)以(yi)(yi)回(hui)陽(yang)(yang),陽(yang)(yang)復有根(gen)有源(yuan)有序(xu);氣(qi)以(yi)(yi)生(sheng)(sheng)津(jin)(jin),津(jin)(jin)以(yi)(yi)和陰;與附子、干姜(jiang)相用,益(yi)氣(qi)回(hui)陽(yang)(yang)生(sheng)(sheng)津(jin)(jin)。《傷寒(han)纘論》曰:“其所以(yi)(yi)用人參(can)(can)者,不(bu)特護(hu)持(chi)津(jin)(jin)液(ye),兼陽(yang)(yang)藥得(de)之,愈(yu)加得(de)力耳(er)。”《本草經疏》曰:“人參(can)(can)能回(hui)陽(yang)(yang)于垂絕,卻虛邪于俄頃,其主(zhu)治(zhi)也(ye),則補(bu)五(wu)(wu)臟,蓋臟雖有五(wu)(wu),以(yi)(yi)言乎(hu)生(sheng)(sheng)氣(qi)之流(liu)通則一也(ye),益(yi)真氣(qi),則五(wu)(wu)臟皆補(bu)矣。”方見四逆加人參(can)(can)湯中(zhong),主(zhu)治(zhi)心腎(shen)陽(yang)(yang)氣(qi)虛弱(ruo)或欲脫陰損證。

1.19益氣生營血:本品益(yi)氣(qi),氣(qi)以(yi)生(sheng)血(xue),血(xue)以(yi)和營(ying),營(ying)以(yi)泌血(xue);與(yu)芍藥相用(yong),以(yi)增(zeng)益(yi)氣(qi)化營(ying)血(xue)。《傷寒論(lun)辨證廣注》曰:“加人參者,陽生(sheng)陰長,甘能益(yi)血(xue)也。《本草正(zheng)》曰:“人參,氣(qi)虛(xu)血(xue)虛(xu)俱(ju)能補。”方(fang)見桂枝新加湯中(zhong),主治太陽中(zhong)風證與(yu)營(ying)血(xue)不足證相兼(jian)。

1.20益氣生血和陰陽:本品益(yi)氣而(er)(er)(er)生(sheng)(sheng)血(xue)(xue),助陽以和(he)陰,善補脾益(yi)心榮肺和(he)肝盈(ying)元氣,安神定志而(er)(er)(er)藏魂魄,撫思(si)慮而(er)(er)(er)止煩悸,通補五臟,以補氣為先(xian),生(sheng)(sheng)血(xue)(xue)為次,氣血(xue)(xue)得補而(er)(er)(er)陰陽和(he)也方見薯蕷丸中,主治(zhi)陰陽氣血(xue)(xue)俱不足證或兼太陽病證。

1.21益氣化飲保胎:本品補益(yi)脾(pi)胃,使脾(pi)胃功能(neng)復常以(yi)(yi)生化(hua)(hua)氣血,氣血生化(hua)(hua)有(you)常以(yi)(yi)榮胎(tai),溫中(zhong)而能(neng)散除寒濕,中(zhong)氣得(de)(de)健則飲(yin)邪不生;與干姜、半(ban)夏相用,溫中(zhong)有(you)化(hua)(hua),降中(zhong)有(you)補,化(hua)(hua)飲(yin)降逆不傷(shang)胎(tai)氣或(huo)中(zhong)氣,并使中(zhong)氣得(de)(de)和,胎(tai)氣得(de)(de)保(bao)。方見干姜人參半(ban)夏丸中(zhong),主(zhu)治(zhi)妊娠(shen)脾(pi)胃虛(xu)寒飲(yin)逆證(zheng)。

1.22益宗氣瀉飲邪:本品(pin)補益宗(zong)氣(qi),使(shi)宗(zong)氣(qi)貫心(xin)脈,行津液(ye),宗(zong)氣(qi)不虛,飲(yin)邪不生(sheng);與木(mu)防(fang)相用,既可(ke)使(shi)已成之飲(yin)邪得去(qu),又(you)可(ke)斷絕飲(yin)生(sheng)之源。方(fang)見木(mu)防(fang)己湯中(zhong)。主(zhu)治膈間陽(yang)郁熱飲(yin)證;方(fang)見木(mu)防(fang)己去(qu)石膏(gao)加茯苓芒硝湯中(zhong),主(zhu)治膈間陽(yang)郁水飲(yin)結(jie)重證。

1.23 益氣行血祛瘀:本(ben)品益氣(qi),氣(qi)為血(xue)之帥,氣(qi)行(xing)則血(xue)行(xing),血(xue)行(xing)則瘀(yu)血(xue)去(qu);與活血(xue)之品相(xiang)用(yong),益氣(qi)以(yi)(yi)行(xing)血(xue),理血(xue)以(yi)(yi)祛瘀(yu);入(ru)胞中則益氣(qi)祛瘀(yu),生新(xin)榮(rong)胞。方見溫經(jing)湯中,主治(zhi)婦(fu)人宮寒(han)血(xue)瘀(yu)諸證;方見鱉甲煎丸(wan)中,主治(zhi)“瘧母”,“癥瘕。”
